Claims and Billing
Network Providers
Network providers submit your claims directly to VSP and receive payment in return. If your provider is allowed to charge you for the difference between what the plan will pay and the discounted fee, he or she may request that portion of the payment at the time of service or bill you later. An Explanation of Benefits will describe how the claim was processed.
Non-Network Providers
If you use a non-network provider, you are responsible for completing a claim form and sending it to VSP for reimbursement. Please send claim forms to:
